Chronic Care: A Palliative Care Service

The Mission of the Chronic Care Team at Lafayette General is to improve the quality of life for chronically ill patients through disease management services.

What is Chronic Care?

Patients and their families, throughout the challenges of chronic illness, often face hope and fear. The hope for relief of symptoms, more time with their loved ones, and quality of life. They fear pain, suffering and death. Chronic Care, is a model of care for patients with all types of chronic, progressive illnesses that focuses on maintaining quality of life throughout the course of the disease.

The purpose of the Chronic Care Team is to:

  • Collaborate with the primary care physician regarding symptom management
  • Assist in alleviating psychosocial, emotional and spiritual suffering
  • Achieve the best quality of life possible for patients
  • Positively influence the course of chronic disease through education
  • Support patients' families
  • Provide assistance with making difficult end-of-life decisions

Q: can I continue to receive treatments from my current team pf physicians?
A: Yes. We work together with your physicians to individualize your care.
 
Q: Is chronic/palliative care the same as hospice care?
A: No. Chronic/palliative care provides support and comfort at any stage of a serious illness, whether 
     terminal or not.
 
Q: How can a patient get Chronic Care?
A: The initial evaluation can be requested by any patient, family member or healthcare professional.
